1. By participating, I accept all risks necessarily following from my participation that could result in loss of life or permanent injury. Accordingly, I release all persons, associations and corporations associated directly or indirectly with the conduct of the sport of judo from all claims demands and proceedings arising out of my participation and I hereby indemnify them against all liability (including liability for their negligence and the negligence of others) for all injury, loss or damage arising out of or connected with my participation in the sport of judo.   This release shall extend to and include Judo Federation of Australia (Qld) Inc., Shin Gi Tai Pty Ltd and their respective directors, partners, managers, officers, agents, contractors, employees and volunteers including medical and paramedical personnel appointed for the sport of judo, the owners, licensees and occupiers of land upon which the sport of judo or any part of it is conducted, any statutory body or local authority having control over land upon which the sport of judo or any part of it is conducted or which is involved directly or indirectly with the sport of judo in any manner whatsoever and promoters, sponsors and event organisers. This release and indemnity continues forever and binds my heirs, executors, personal representatives and assigns.

2. I consent to receiving any medical assessment and or treatment including ambulance transportation that the Judo Federation of Australia (Qld) Inc or Shin Gi Tai Pty Ltd organisers think desirable during or after my participation in the sport of judo.

3. I consent to the Judo Federation of Australia (Qld) Inc or Shin Gi Tai Pty Ltd organisers using my name, image and likeness before, during or after my participation in the sport of judo for promotional broadcasting or reporting purposes in any media.

4. I understand that compulsory insurance cover for participants in the sport of judo may not cover me for any or all injury, loss or damage sustained by me.

5. Safety precautions undertaken by Judo Federation of Australia (Qld) Inc or Shin Gi Tai Pty Ltd organisers are a service to me and other competitors but are not a guarantee of safety.

6. I am fully responsible for the security of my personal possessions whilst participating in the sport of judo.

7. My membership is not transferable to other people. If I am unable to take part in the sport of judo, my membership registration fee is non-refundable.

8. I have listed below my medical or physical conditions from which I suffer that might affect my participation in the sport of judo or be relevant if medical treatment is needed. I accept the risk of participating in the sport of judo, despite these conditions.

9. I agree to abide by all rules and directions and codes of conduct issued by the Judo Federation of Australia (Qld) Inc or Shin Gi Tai Pty Ltd.

10. The Judo Federation of Australia (Qld) Inc or Shin Gi Tai Pty Ltd organisers may change the format of my participation in the sport of judo, or other conditions at their discretion. If that occurs, this agreement applies to the changed conditions.

11. I acknowledge that participation in the sport of judo involves the real risk of serious injury or even death from various causes including overexertion, equipment failure, dehydration, and accidents with other participants, spectators and other causes.

12. I declare that I have no known pre existing medical or physical condition that results in greater risk to my welfare, or the welfare of others, participating in the sport of judo. If a known pre-existing medical or physical condition exists I have declared these to the Management Committee in a letter accompanying this application.

13. I understand that I should not take part in the sport of judo or any training activity unless I have trained appropriately and a medical practitioner has verified my physical condition if I am affected by a pre-existing medical or physical condition.

14. I agree that I have read, understand and accept the terms and conditions set out above.
